Office hours
Our office is open Monday through Friday from 8:30 a.m. to 4:30 p.m. We are often out of the office at home visits, advocating for our elders, attending meetings, trainings or driving someone to the doctor. Please leave a phone or emal message and someone will communicate with you as soon as possible.  Our nurse is not in the office on Mondays and Fridays. We do not do home visits or staff the office at the weekend. 
  
Our history
The Payne-Phalen Living at Home/Block Nurse Program housed in First Covenant Church at 1280 Arcade St., St. Paul, was established in 1999 by members of the church and the local community. We became a 501(C)(3) non-profit organization in 2004.
 
We are one of 42 Living at Home/Block Nurse Programs in Minnesota and one in North Dakota serving seniors in a specific geographical location. Our program's uniqueness reflects the diverse community we serve in the District 5 Planning area on St. Paul's East Side.
                                                                                

Who we serve

Currently, we serve over 200  low and moderate income elders who are 65 and older (and their families) on or near the East Side of St. Paul.
  

Volunteers

Much of the support for our program is provided by our dedicated volunteers including the Board of Directors, service learning nursing students from area colleges and the university, and community members who serve as drivers, friendly visitors, and provide office assistance.

 

Mission

Our program's mission: We build a circle of care around the diverse elders on St. Paul's East Side keeping them healthy and safe in their homes.

  

Saving Taxpayers $$$

In 2008, the program kept 23 elders out of nursing homes for 209 months at an estimated savings to taxpayers of over $1 million. (From the Cost & Services Analysis Report found on www.elderberry.org)

Blood Pressure Clinics
We have monthly Blood Pressure Clinics. Please call us at 651.774.7078 to find the one that fits your needs.   
 

Among our educational partners are the University of Minnesota School of Nursing, Metropolitan State University, Century and Inver Hills Colleges Nursing Program. We currently are a site for service learning nursing and other students who are volunteers matched with elders. The elders serve as mentors to the students teaching them the art of aging.

  

Recognition

In 2004, the program was one of six organizations in Minnesota that were recognized and honored by the Jimmy and Rosalynn Carter Service Learning Award.
